Services Available

Assessment: helps to make realistic decisions about your future with the help of the in-house team of professionals

Rehabilitation: to help retain, regain or develop your skills

Short breaks: if you need a break from looking after yourself or if your regular carers need a break

Day care: if you live in the local community and need some regular or occasional extra help

We hope that your decision to use any of these services will be a positive, personal choice. Although our centres do not provide nursing care, with the help of community health care staff, we offer care and support if you have:

• Recently been in hospital or are awaiting discharge
• Poor physical health
• A physical disability or problems with eyesight or hearing
• Memory loss, mild confusion or depression
• Had an accident or an illness, or you are unable to manage at home for a particular reason.

Before you and the people who care for you make any decisions, we will talk about the support you need and want, so everybody concerned can be sure that this is the right decision for you.
