What you can expect from the intermediate care unit

An individually tailored rehabilitation care plan to help you achieve your goals. These could be working towards regaining your strength, confidence and independence with:

  • Mobility – this will incorporate an exercise  programme that you agree with the Physiotherapist. Rehabilitation Support Assistants will then enable you to follow your programme in our equipped gym.
  • Drinks and meals – The Occupational Therapist and Rehabilitation Support Assistants will work with you in our specially adapted kitchen. This will enable you to practice kitchen skills and work towards regaining your independence with the help of special aids if required.
  • Washing, dressing and bathing – support and guidance can be given with the help of specialist equipment if needed, to enable you to become independent with some or all of your personal care.
  • Managing at your own home – the Occupational Therapist will escort you home, assess your individual needs for aids such as: grab rails, raised toilet seats. This will help maintain your safety and independence at home when you are discharged from the unit.
  • Nursing needs – a qualified nurse is available when required and will liaise closely with your G.P.
  • Managing your medication – we can assist and work with you towards managing your medication safely.

The rehabilitation service is available free of charge to you for a maximum of 6 weeks. Dependent on your progress and your needs you may need less than 6 weeks.

  • Assessment – Care staff and the Occupational Therapists will work with you to assess your individual needs and abilities.
  • Washing, Dressing and bathing – Occupational Therapist and Care staff will give support and guidance with the  help of specialist equipment if needed, to help you manage some or all of your personal care.
  • Home visit – the Occupational Therapist will escort you home if appropriate to assess your individual needs for aids such as grab rails, raised toilet seats. This will help maintain your independence and safety at home.
  • Medication – we can assist you and work with you towards managing your medication safely.
  • Nurse and Physiotherapist – can be provided for you   if needed by the community teams.

The assessment service is available to you free of charge for a maximum of 6 weeks. Dependent on your needs and choices you may need less than 6 weeks.

Discharge Planning

A small informal meeting called a review will be held at an agreed time with you. This is the time for you to tell us what you want to do following your stay on the rehabilitation or assessment unit. You may invite anyone who you may wish to attend. Your Social Worker will also be invited to support you. At the review we will discuss with you, your progress, we will also advise and put forward recommendations to enable you to maintain your level of independence when you are discharged from the unit.
